Safety Trained Supervisor Life Critical Activities Questions and Answers 1 — Questions and Answers
Question 1: A supervisor is preparing for a job that involves multiple life critical activities, including confined space entry and hot work. Which of the following is the most critical administrative control to implement before work begins?
- Conducting a toolbox talk on general site safety.
- Ensuring all workers have standard PPE.
- Implementing a comprehensive Permit-to-Work (PTW) system. (Correct answer)
- Scheduling the work during daylight hours only.
Correct answer: Implementing a comprehensive Permit-to-Work (PTW) system.
A Permit-to-Work (PTW) system is a formal, documented procedure used to control high-risk activities. For life critical tasks like confined space entry and hot work, a PTW ensures that all hazards have been identified, risks assessed, control measures implemented, and authorizations are in place before work starts. While toolbox talks and PPE are important, the PTW system is the most critical administrative control that integrates all safety checks for such high-risk, concurrent tasks.
Question 2: Which of the following is a primary characteristic of a 'Life Critical Activity' in a workplace safety program?
- The activity is performed on a daily basis by most employees.
- Failure to follow procedures could foreseeably lead to serious injury or fatality. (Correct answer)
- The activity requires a minimum of two people to complete.
- It is a task that has resulted in a minor first-aid case within the last year.
Correct answer: Failure to follow procedures could foreseeably lead to serious injury or fatality.
Life Critical Activities (also known as Life Saving Rules) are tasks that have a high potential for causing serious injury, permanent impairment, or death if safety procedures are not strictly followed. The defining characteristic is the severity of the potential outcome, not the frequency of the task or the number of people involved.
Question 3: A team is scheduled to perform maintenance inside a large industrial tank. According to life critical safety standards, what action must be taken before any worker enters the tank?
- The worker must sign a liability waiver.
- The atmosphere inside the tank must be tested for oxygen levels and hazardous gases. (Correct answer)
- The supervisor must visually inspect the tank from the outside.
- The maintenance task must be completed within 30 minutes.
Correct answer: The atmosphere inside the tank must be tested for oxygen levels and hazardous gases.
Entering a confined space, such as a tank, is a life critical activity. A fundamental and mandatory precaution is to test the atmosphere to ensure adequate oxygen levels (typically above 19.5%) and the absence of toxic or flammable gases before entry. This step is crucial to prevent asphyxiation or poisoning.
Question 4: An electrician is tasked with replacing a faulty breaker in a high-voltage panel. This task is considered a life critical activity primarily because it involves:
- Working in a physically awkward position.
- The potential for exposure to hazardous energy. (Correct answer)
- The use of specialized, expensive tools.
- Coordination with the facility's power company.
Correct answer: The potential for exposure to hazardous energy.
Electrical work on high-voltage equipment is a life critical activity because of the severe risk of exposure to hazardous energy (electricity). The primary life-threatening hazard is electric shock, arc flash, or electrocution. Proper energy isolation procedures, like Lockout/Tagout, are essential to control this hazard.
Question 5: A supervisor observes a rigger preparing to lift a heavy load near a designated walkway. To comply with life critical rules for lifting operations, which of the following is the most important immediate action?
- Verifying the rigger is wearing a hard hat and safety glasses.
- Ensuring the crane has had its annual inspection.
- Establishing a controlled zone and preventing personnel from walking under the suspended load. (Correct answer)
- Checking the weather forecast for potential high winds.
Correct answer: Establishing a controlled zone and preventing personnel from walking under the suspended load.
A key life critical rule for lifting and rigging is to keep people out of the 'line of fire'. This involves controlling the area and ensuring no one is under the suspended load, as a failure could be catastrophic. While PPE, inspections, and weather are all important safety considerations, preventing personnel from being under the load is the most critical and immediate action to prevent a fatality.
Question 6: Which of the following activities would MOST likely be governed by a company's 'Life Critical' safety standards?
- Using a desktop computer in an office.
- Manually lifting a 20-pound box in a warehouse.
- Performing excavation work in a 6-foot deep trench. (Correct answer)
- Sweeping the floor in a workshop.
Correct answer: Performing excavation work in a 6-foot deep trench.
Excavation and trenching are recognized high-risk activities that are typically included in life critical standards. A trench 6 feet deep poses a significant risk of collapse and engulfment, which can be fatal. This contrasts with the other activities listed, which have a much lower risk of causing a life-threatening injury.
