QMAP training and certification requirements vary by state but typically involve: an approved QMAP training course (8-32 hours depending on state), skills competency demonstration, passing a written exam, and working under supervision of a licensed nurse. QMAP-certified staff can administer oral, topical, eye, ear, and inhaled medications as defined by state regulations.

QMAP-certified caregivers are generally authorized to administer: Oral medications (tablets, capsules, liquids), Topical medications (creams, ointments, patches), Ophthalmic medications (eye drops, eye ointments), Otic medications (ear drops), Inhaled medications (metered dose inhalers with spacers, nebulizer treatments), and Rectal medications (suppositories in some states). QMAPs typically CANNOT administer: injections (insulin, IM), IV medications, controlled substances in many states, or medications requiring clinical judgment. Always check state-specific scope of practice regulations.

The Five Rights of Medication Administration are the foundational safety check every QMAP must perform before giving any medication: 1) Right Patient — verify the patient's identity using two identifiers. 2) Right Medication — confirm the drug name matches the MAR (Medication Administration Record). 3) Right Dose — verify the amount ordered matches what is being given. 4) Right Route — confirm the prescribed administration route (oral, topical, etc.). 5) Right Time — give medication at the scheduled time. Many facilities now teach additional rights: Right Documentation (document immediately after giving), Right Reason, and Right to Refuse.

Proper QMAP documentation on the Medication Administration Record (MAR) includes: Date and time of administration, Name and dose of medication administered, Route of administration, QMAP's signature or initials, Any special observations or resident responses. Errors must never be erased — draw a single line through the error, write "ERROR," initial, and document correctly. Omissions (medication not given) must be documented with reason codes. Controlled substances require additional documentation including count records. Accurate MAR documentation is both a legal requirement and critical for resident safety.

QMAPs must recognize common side effects and report them to supervising nurses: Sedatives/hypnotics (Ambien, Benadryl) — excessive drowsiness, confusion, fall risk. Antihypertensives (metoprolol, lisinopril) — dizziness, orthostatic hypotension (fall risk when standing). Diuretics (furosemide) — frequent urination, dehydration, electrolyte imbalances. Anticoagulants (warfarin) — increased bleeding risk, bruising. Antipsychotics — extrapyramidal symptoms (muscle stiffness, tremors, tardive dyskinesia). QMAPs must never make medication changes independently — all concerns are reported immediately to the supervising nurse.

Controlled substances (narcotics, stimulants, sedatives — DEA Schedules II-V) require stricter oversight than non-controlled medications. QMAP responsibilities typically include: Performing and documenting controlled substance counts at each shift change (witnessed count with another staff member), Storing controlled substances in a double-locked cabinet, Documenting each dose administered on both the MAR and controlled substance log, Reporting any discrepancies immediately to the supervising nurse, and Never leaving controlled substances unattended. State laws and facility policies govern whether QMAPs can access controlled substances at all — some states restrict this to licensed nurses only.
