MoCA Orientation to Time

Question 1

How many of the MoCA orientation items specifically assess temporal orientation?

Accepted Answer

Four (date, month, year, day of week)

Answer

Four of the six orientation items assess temporal orientation: the exact date, the month, the year, and the day of the week. The remaining two (place and city) assess spatial orientation.

Question 2

A patient correctly states the year and month but cannot identify the specific date or day of the week. How many temporal orientation points do they earn?

Accepted Answer

2 points

Answer

Year (1 pt) + month (1 pt) correct, date (0 pt) + day of week (0 pt) incorrect = 2 out of 4 temporal orientation points.

Question 3

Why is knowing the correct day of the week significant in the MoCA orientation section?

Accepted Answer

Because it is a sensitive early marker of temporal disorientation, especially in hospitalized patients

Answer

Day of week orientation reflects short-term contextual tracking. Many hospitalized or institutionalized patients lose track of the day before losing track of the month or year, making it a sensitive early marker.

Question 4

Which temporal orientation item is typically lost LAST in progressive dementia?

Accepted Answer

Year

Answer

Year orientation is typically the last temporal orientation item to be lost in dementia. Patients lose track of the specific date and day first, then month, and finally year as dementia progresses.

Question 5

When a patient is 1 day off on the MoCA date (e.g., says the 14th when it is the 15th), the date orientation is scored as:

Accepted Answer

0 points — exact date is required

Answer

MoCA date orientation requires an exact response. Being off by even 1 day results in 0 for that item. There is no partial credit or tolerance for errors on this task.
