Free CRST Safety Management Systems Questions and Answers — Questions and Answers
Question 1: A key feature of the Canadian standard CSA Z45001, which distinguishes it from the international ISO 45001 standard, is its specific emphasis on:
- Financial auditing of safety programs.
- The Plan-Do-Check-Act (PDCA) model.
- The active participation and rights of workers and their representatives. (Correct answer)
- Management-only responsibility for incident investigation.
Correct answer: The active participation and rights of workers and their representatives.
CSA Z45001 is an adoption of ISO 45001 but includes specific Canadian deviations. A primary deviation is the enhanced emphasis on worker participation, recognizing workers' rights (like the right to refuse unsafe work), and ensuring the involvement of worker representatives in OHS management decision-making.
Question 2: A manufacturing company decides to replace an old, manually operated press with a new, fully automated one. Within their Safety Management System, which formal process must be initiated BEFORE this change is implemented?
- A post-implementation safety audit.
- Management of Change (MOC). (Correct answer)
- An annual performance review.
- A lagging indicator analysis.
Correct answer: Management of Change (MOC).
Management of Change (MOC) is a systematic process used to ensure that risks arising from changes to facilities, equipment, technology, or procedures are properly identified, evaluated, and controlled before the change is implemented. Replacing a key piece of equipment is a significant change that triggers the MOC process.
Question 3: Which of the following is the best example of a 'leading indicator' for measuring the performance of a Safety Management System?
- The number of lost-time injuries in the previous quarter.
- The company's workers' compensation premium rate.
- The percentage of required safety inspections completed on schedule. (Correct answer)
- The total number of recordable incidents for the year.
Correct answer: The percentage of required safety inspections completed on schedule.
Leading indicators are proactive, preventive, and predictive measures that monitor activities designed to prevent incidents. The percentage of completed safety inspections is a measure of a proactive safety activity. The other options are all lagging indicators, which are reactive and measure outcomes after an incident has occurred.
Question 4: In the 'Plan-Do-Check-Act' (PDCA) cycle for continual improvement within a safety management system, what key activity occurs during the 'Check' phase?
- Implementing new safety procedures and training.
- Identifying hazards and setting OHS objectives.
- Senior management taking action to correct deficiencies.
- Monitoring and measuring performance against OHS policy and objectives. (Correct answer)
Correct answer: Monitoring and measuring performance against OHS policy and objectives.
The 'Check' phase of the PDCA cycle involves monitoring and measuring activities and processes against the OHS policy, objectives, and legal requirements, and reporting the results. This assessment is crucial for determining if the 'Plan' and 'Do' stages were effective before moving to the 'Act' stage.
Question 5: Following a workplace incident, an investigation is conducted as part of the Safety Management System. What is the primary goal of this investigation?
- To assign blame to the employee(s) involved.
- To satisfy the minimum requirements of insurance providers.
- To identify the root cause(s) to prevent recurrence. (Correct answer)
- To complete regulatory paperwork within 24 hours.
Correct answer: To identify the root cause(s) to prevent recurrence.
The fundamental purpose of an incident investigation within a safety management system is not to assign blame, but to identify the root causes—the underlying system failures—that allowed the incident to happen. By addressing the root cause(s), the organization can implement effective corrective actions to prevent similar incidents in the future.
Question 6: A construction firm is implementing a formal Safety Management System (SMS). According to modern safety principles, which element is most critical for the success and sustainability of the SMS?
- A comprehensive library of safety forms and checklists.
- The purchase of the latest personal protective equipment (PPE).
- A detailed disciplinary policy for safety infractions.
- Demonstrated and visible commitment from senior leadership. (Correct answer)
Correct answer: Demonstrated and visible commitment from senior leadership.
While all elements can be part of an SMS, the visible commitment and leadership from top management is universally recognized as the most critical factor for its success. Leadership drives the safety culture, allocates resources, and ensures that safety is integrated into all business decisions, making the entire system effective and sustainable.
