As noted in Box 11, birth certificate, credit cards, and social security cards are unacceptable as identification.
The applicant must sign the form in Box 15. The postal representative does not sign the form.
Box 9 requires the postal representative to verify ID and Physical Address. The applicant’s name is verified as part of the ID verification.
Box 5 requires an address. 4/16/2008 is a date so it would not belong in Box 5.
This is a tricky question. Box 8 indicates the box size needed. Box 13 indicates which type of service is assigned (i.e., Box). Box 2 is not correct because it merely lists the numbers but does not indicate that a Box or Caller service was assigned.
If the application is filled out for personal use Box 1 and 3 are filled in.
If an individual is applying for their company, they would put their name and title in Box 3.
The customer should fill out all unshaded boxes (1,3-6,14-15)
Box 7 requires a date to be entered.

A checkmark would be placed by HCR in Box 12.
The size of the PO Box is indicated in Box 8.
As indicated in the directions in Box 11, a birth certificate is not an accepted form of identification.
