What is a hazard assessment, and when should it be performed?
Answer: A systematic evaluation of workplace conditions to identify hazards and determine required controls, performed before work and whenever conditions change
A hazard assessment is a proactive evaluation conducted before starting work and whenever conditions change (new equipment, process changes, incidents), identifying all potential hazards and specifying the controls needed to protect workers.
What are the requirements for a hot work permit in an industrial environment?
Answer: Fire hazards must be assessed, combustibles removed or protected, fire watch established, atmospheric monitoring conducted if needed, and the permit signed by authorized personnel before any welding, cutting, or grinding begins
Hot work permits require: identification and removal of fire hazards, protection of remaining combustibles, atmospheric testing for flammable vapors, fire extinguisher availability, designated fire watch during and after work, and written authorization.
What PPE is required for a millwright performing grinding operations?
Answer: Safety glasses or face shield, hearing protection, leather gloves, and appropriate clothing to protect against sparks and flying debris
Grinding generates high-velocity particles, sparks, and significant noise. Required PPE includes impact-rated eye protection (safety glasses at minimum, face shield recommended), hearing protection, gloves, and clothing that covers exposed skin.
What is the hierarchy of controls, and why is it important in hazard management?
Answer: A ranked system of hazard controls from most effective (elimination) to least effective (PPE), prioritizing controls that remove the hazard over those that protect the worker

What should a worker do if they discover an unsafe condition on a job site?
Answer: Immediately report it to their supervisor and stop work in the affected area if there is an imminent danger to any worker
Workers have both the right and the duty to report unsafe conditions immediately. If the hazard presents imminent danger, workers must stop work in the affected area and notify their supervisor and safety personnel right away.
What are the key elements of a workplace emergency response plan?
Answer: Evacuation routes and assembly points, emergency contacts, alarm systems, roles and responsibilities, first aid provisions, and regular drills
A comprehensive emergency response plan includes: identified hazards and scenarios, evacuation procedures and routes, assembly points, alarm systems, emergency contacts, assigned roles, first aid/rescue capabilities, communication protocols, and regular training drills.
