Lean Six Sigma Yellow Belt Certification FREE Lean Six Sigma Yellow Belt Define Phase Deliverables Questions and Answers 2 — Questions and Answers
Question 1: What is the primary purpose of a Project Charter in the Define phase of a Lean Six Sigma project?
- To formally authorize the project and define its scope, objectives, and resources (Correct answer)
- To document the root cause of a problem
- To create a detailed implementation plan
- To measure current process performance
Correct answer: To formally authorize the project and define its scope, objectives, and resources
The Project Charter formally authorizes the project and establishes the scope, objectives, timeline, and team resources for stakeholder alignment.
Question 2: Which Define phase deliverable maps the end-to-end process at a high level using Suppliers, Inputs, Process steps, Outputs, and Customers?
- Value Stream Map
- SIPOC Diagram (Correct answer)
- Control Chart
- Fishbone Diagram
Correct answer: SIPOC Diagram
A SIPOC Diagram provides a high-level overview of the process by identifying Suppliers, Inputs, Process steps, Outputs, and Customers.
Question 3: What does the Voice of the Customer (VOC) translate into for use in a Lean Six Sigma project?
- Key Performance Indicators (KPIs)
- Critical to Quality (CTQ) requirements (Correct answer)
- Standard Operating Procedures (SOPs)
- Failure Mode and Effects Analysis (FMEA)
Correct answer: Critical to Quality (CTQ) requirements
VOC data is translated into Critical to Quality requirements that define measurable standards the process must meet to satisfy customers.
Question 4: A well-written problem statement in the Define phase should avoid which of the following?
- Quantifying the magnitude of the problem
- Describing the impact on the business
- Suggesting a solution or root cause (Correct answer)
- Identifying when and where the problem occurs
Correct answer: Suggesting a solution or root cause
A proper problem statement describes what is wrong without prescribing a solution or implying a root cause, keeping the investigation unbiased.
Question 5: Which stakeholder analysis tool categorizes stakeholders by their level of influence and interest in a project?
- RACI Matrix
- Power-Interest Grid (Correct answer)
- Affinity Diagram
- Pareto Chart
Correct answer: Power-Interest Grid
The Power-Interest Grid plots stakeholders based on their level of influence and interest to help determine appropriate engagement strategies.
Question 6: What is the purpose of defining CTQ (Critical to Quality) trees during the Define phase?
- To rank defects by frequency of occurrence
- To break down broad customer needs into specific, measurable requirements (Correct answer)
- To calculate process sigma levels
- To identify all possible failure modes
Correct answer: To break down broad customer needs into specific, measurable requirements
CTQ trees decompose high-level customer needs into specific, actionable, and measurable quality requirements that the team can target.
