Finally passed FLS after failing twice — here's what actually helped

So I just got my results this morning and I actually passed the FLS exam on my third attempt. I'm a PGY-2 surgical resident and honestly the first two times I just tried to wing it with some quick reading. Big mistake. The pharmacology and pharmacokinetics sections absolutely destroyed me — I had no idea how deep they went into drug metabolism pathways.

What finally turned it around was committing to a structured study guide instead of just skimming review PDFs. I spent about 6 weeks, roughly 45 minutes a day, and I leaned heavily on FLS practice test questions to figure out where my gaps actually were. Doing timed blocks exposed that I was consistently missing questions on opioid equivalencies and reversal agents, which I then drilled hard.

For anyone currently prepping: don't underestimate the practical component either. I almost over-studied the written stuff and nearly forgot to review the manual skills criteria. Happy to answer questions about the written portion specifically if anyone's in the thick of it.

Congrats! Third time is no joke — that takes real persistence. I sat for mine last spring and the pharmacology section caught me off guard too. My biggest exam tip would be to actually time yourself on practice blocks from the start, not just read through questions casually. I noticed my accuracy dropped about 12% under timed conditions early on, so getting used to that pressure made a real difference by test day.

Can I ask which study guide you used? I've been bouncing between a couple of resources and nothing feels comprehensive enough for the drug content. I'm about 4 weeks out from my exam date and starting to panic a little. Also did you find the actual exam matched the difficulty level of the practice questions you were doing, or was it harder? That's my biggest fear going in honestly.

The opioid equivalency questions are no joke — spent way too long making a conversion chart and just drilling it until it was automatic. Definitely worth the time. Good luck to everyone prepping right now, it's a passable exam if you're methodical about it.
