Finally passed ECMO specialist exam after two attempts — what actually helped

So I just got my results back and I'm officially a certified ECMO specialist. Took me two tries, and honestly the first attempt was embarrassing — I walked in thinking my clinical experience would carry me and completely underestimated the pharmacology and circuit troubleshooting sections. Scored a 68 and needed a 70. Gutting.

For round two I completely changed my approach. I spent about six weeks on a structured study guide this time, focusing heavily on gas exchange physiology, anticoagulation management, and emergency interventions. I also drilled with an ECMO practice test almost every night — not to memorize answers but to get comfortable with how the questions are framed. The ELSO guidelines are dense but they're basically the bible for this exam.

Happy to share what resources actually moved the needle for me versus what felt like busy work. Anyone else prepping right now or have questions about specific topic areas? The circuit management questions are sneakier than they look on paper.

Congrats on passing! I'm about 4 weeks out from my exam date and the anticoagulation section is killing me. There's so much variation between institutions in real practice that I keep second-guessing what the "correct" textbook answer is. Did you find the exam stuck pretty closely to ELSO guidelines, or were there curveball scenarios that went beyond that?

Two attempts is nothing to be ashamed of — this exam has a lower first-time pass rate than most people expect going in. I used a combination of the ECMO specialist study guide from the Extracorporeal Life Support Organization and a few practice test sets I found online. What helped me most was explaining concepts out loud to myself. If I couldn't teach it, I didn't actually know it yet.

The exam tips about pacing yourself are real — I spent way too long on the first few questions and had to rush the back half. Flag and move on. You can always come back. Good luck to everyone still prepping.
