COMSAE score interpretation — what does a 500 actually mean for Step 2?

Just got back my COMSAE Phase 2-CE results and scored a 500. I'm an OMS-3 starting rotations and trying to figure out how predictive this is for my actual COMLEX Level 2-CE score. The scoring scale is confusing to me and I can't find a consistent answer on what a 500 translates to in terms of likelihood of passing the real exam.

I've heard COMSAE is supposed to be correlated with COMLEX performance but I don't know how reliable that correlation actually is or how big the standard error is. Some people in my class say it usually underpredicts your actual score, others say it's pretty accurate. One person said their COMSAE was 30 points lower than their actual COMLEX, another said they were within 10 points. That range makes it hard to know how to interpret my own result.

I've got about 10 weeks before my Level 2 date. My shelf scores have been in the 70 to 75% range across rotations, which I think is decent, but I'm not sure how to triangulate all of these data points together. The osteopathic principles section of COMSAE is where I dropped the most points and I've always found that material harder to retain than the clinical content.

Should I be concerned about a 500, or is that a reasonable baseline to build from with 10 weeks of focused prep? Anyone who's used COMSAE as a diagnostic and then sat for the real Level 2 — how close were the scores?

A 500 at 10 weeks out with solid shelf scores is not a crisis. Most people I know who passed comfortably had COMSAE scores in that range at the same point in their prep. The exam itself has enough clinical overlap with your rotation knowledge that your shelf performance is actually a meaningful data point.

10 weeks is a lot of runway. I started my serious Level 2 prep at 8 weeks with a similar baseline score and ended up fine. Don't spiral on the number — use it diagnostically and make a plan for the specific areas that need work.

The standard error on COMSAE is pretty wide so don't put too much weight on the exact number. What it's more useful for is identifying which content domains you're weak in. If OPP is where you're dropping points, that's your signal to spend the next few weeks specifically on that content.

My COMSAE was 487 and I scored 527 on the actual COMLEX. That's anecdotal but it tracks with the general pattern I've heard — COMSAE tends to run slightly lower than your real score for most people, not higher.

The OMM content is worth drilling specifically. It's a disproportionate score opportunity on the real exam.
